When dealing with porosity the first place to look is a compromise of the shielding medium. It's very common in MIG welding when the gas pressure runs low or the tip gets clogged and in arc welding and brazing if the flux on the outside of the rod gets cracked or chipped. Moisture is a common cause of this, so make sure your rods are in good shape and store them in a warm dry place.
