There is a You Tube video of some guys working a diesel engine that goes
into run away.  They try choking off the intake too.

I replaced a bad Jake Brake switch on a fire engine one day. I didn't
realize they had to be adjusted so I just screwed it in until it stopped. 
When we fired it up, it was at wide open throttle. Being compression
ignition with no emergency shut off, we couldn't shut it down. So there I am
with a big diesel engine running up against the governor and I couldn't
figure out how to kill it down. The rest of the guys came out of the station
because it sounded like a locomotive was sitting in front of the station. We
tried to shut off the air by putting a plastic bag over the air intake which
almost killed the engine but not quite. It did make the thing smoke like an
old train to the point to where we could barely see! Finally someone found
the fuel shut off under the rig which eventually killed it. I think my blood
pressure was up against the governor until it died. :-]]

Just as scary, I had wide boots when I stepped on the brake in my 89
Wagoneer. The right part of my foot mashing the gas. The back wheels where
squalling and the front wheels dragging as I mashed harder, it kept speeding
up. You instint is NOT to let off the brake. I hit my trailer!  Bill 1362
