I carry a pair of pliers, a tire plug kit and a can of Fix-a-Flat:

I use the pliers to remove the nail or screw that punctured the tire, the plug kit to plug/seal the leak, and the Fix-a-Flat to air it back up if it's too flat to drive safely to a nearby gas station to air up. I actually carry this in each of my cars.
