grab ahold of the flat spade end of the speedo drive gear with a small pair of vice-grip
pliers, then forget about it.  The pliers ensure that it won't slip down through the
center of the sleeve.  I then control the sleeve with one hand and install the bolt with
the other.

Having said that, this job is MUCH more easily done with another hand or two...and you 
happen to have access to additional hands, so why not take advantage of that?
