Yeah, that convert-to-PDF link is more of a problem than it is worth.
I'll get rid of it someday.

There are three grayed-out buttons in the upper right corner of the
form.   
The left one is 'pdf' and it doesn't work.   But the middle one opens
the form 
in a new window, which works fine.   You can then print that.

Alternately, you can get to the same form via a different link on the
old 
POCA website:
