To both questions, make the thing really strong with a wide base: there's a 
surprising amount of torque reaction with a running engine even with gentle 
acceleration. Use big bolts!   Such a thing comes in handy to break in an engine 
so you'll need to run it to at least 2000-2500 rpms to protect the cam lobes. 
Many of the stands I've seen have only a quart or two of fuel supply, just in
